Psychoanalytic psychodrama in France and group elaboration of counter‐transference: Therapeutic operators in play therapy

Abstract

In France, psychoanalytic psychodrama is mainly envisioned in its individual form – that is, a single patient working with a group of therapists. Its originality consists in bringing together several clinicians within a clinical experience that is shared as a group. This experience is fundamentally different from traditional individual therapies, psychotherapies or group co‐led therapies. Its configuration may be confusing or overwhelming due to the large number of co‐therapists involved in the setting. However, thanks to group elaboration based on the transferential‐countertransferential dynamics induced by the treated patient, this potential ‘cacophony’ can lead to fruitful psychic development embedded in play. This is tied to the co‐therapists' positioning in the transitional space shared with the patient as well as to the patient's subjective appropriation of their initiatives. By reflecting on clinical material taken from actual sessions as well as from the exchanges and elaborations occurring at their margins, this article shows how psychodrama and group come to metabolize the transferential elements, shaping the engagement of participants in the context of improvised play. En France, le psychodrame psychanalytique est surtout pratiqué sous une forme individuelle – un seul patient travaille avec un groupe de thérapeutes. Son originalité réside dans la réunion de plusieurs thérapeutes cliniciens qui partagent une expérience clinique en groupe. Cette expérience diffère fondamentalement des thérapies individuelles traditionnelles, ainsi que des psychothérapies ou animations de groupe. Une telle configuration peut se révéler déroutante ou aveuglante en raison de la présence d'un grand nombre de co‐thérapeutes. Cependant, cette « cacophonie » est également propice à la construction psychique au sein du groupe. Cette construction psychique qui émane des multiples mouvements transférentiels et contre‐transférentiels envers le patient, trouve son aboutissement dans une situation de jeu, où l'initiative du patient devient l'objet d'une appropriation subjective de sa part sur fond de partage de l'espace transitionnel avec plusieurs thérapeutes. En offrant quelques pistes de réflexion à partir de la clinique, du matériel extrait de séances et de son élaboration via un travail d'échange d'idées, cet article éclaire la façon dont le psychodrame et le groupe métabolisent les éléments transférentiels tout en plaçant les participants dans un jeu improvisé. In Frankreich kennt man das psychoanalytische Psychodrama vorwiegend in seiner individualisierten Form, d.h., ein einzelner Patient arbeitet mit einer Gruppe von Therapeuten. Die Besonderheit dieses Settings besteht darin, dass mehrere klinische Therapeuten als Gruppe gemeinsam eine klinische Erfahrung machen, die sich grundlegend von traditionellen Einzeltherapien, Psychotherapien oder Gruppenanimationen unterscheidet. Ihre Konfiguration kann aufgrund der Anwesenheit einer großen Zahl von Ko‐Therapeuten verwirrend oder überwältigend sein. Aber diese „Kakophonie” kann sich für eine psychische Konstruktion innerhalb der Gruppe als effektiv erweisen. Diese psychische Konstruktion wird durch multiple Übertragungs‐ und Gegenübertragungsentwicklungen mit dem Patienten koordiniert und führt zum besten Ergebnis in einer Spielsituation, in der sich der Patient seine Initiative subjektiv zu eigen machen kann, während er den Übergangsraum mit mehreren Therapeuten teilt. Der Artikel beschreibt die klinischen Erfahrungen sowie den Austausch über diese besonderen Sitzungen und ihre Bearbeitung und wirft zudem Licht auf die Art und Weise, wie Psychodrama und Gruppe Übertragungselemente verstoffwechseln und die Teilnehmenden in einem improvisierten Spiel positionieren. In Francia, lo psicodramma psicoanalitico è, per lo più, concepito nel suo assetto individuale, che consiste nel lavoro di un paziente con un gruppo di terapeuti. La sua originalità risiede nel riunirsi di diversi terapeuti che condividono una esperienza clinica con il gruppo. Questa esperienza è sostanzialmente diversa dai percorsi individuali, dalle psicoterapie o dall'animazione di gruppo. La sua configurazione può generare confusione o far sentire sopraffatti, a causa della presenza di un gran numero di co‐terapeuti. Tuttavia, questa “cacofonia” può risultare efficace per la costruzione psichica nel contesto di gruppo. Questa costruzione psichica è coordinata insieme al paziente, attraverso molteplici movimenti transferali e contro‐transferali, e porta a un risultato ottimale in una situazione di gioco, nella quale il paziente si appropria soggettivamente della propria iniziativa, pur condividendo lo spazio transizionale con diversi terapeuti. Riflettendo sulle esperienze cliniche, sulla discussione e sull'elaborazione di specifiche sedute, l'articolo si propone di far luce sul modo in cui lo psicodramma e il gruppo metabolizzano gli elementi transferali e contestualmente collocano i partecipanti sulla scena di una improvvisazione teatrale. En Francia, el psicodrama psicoanalítico se concibe principalmente en su modalidad individual, es decir, un solo paciente que trabaja con un grupo de terapeutas. Su originalidad estriba en la confluencia de varios terapeutas que comparten una experiencia clínica como grupo. Esta experiencia es fundamentalmente distinta de las terapias individuales tradicionales, las psicoterapias y la animación grupal. Su configuración puede resultar confusa o abrumadora debido a la presencia de un gran número de co‐terapeutas. Sin embargo, esta “cacofonía” podría resultar eficaz para una construcción psíquica dentro del grupo, que se coordina a través de múltiples movimientos de transferencia y contratransferencia con el paciente. Estos llevan a un mejor resultado, en una situación de juego en la cual el paciente puede apropiarse de su propia iniciativa cuando comparte el espacio transicional con varios terapeutas. Mediante la reflexión sobre experiencias clínicas y una exploración más detallada de esas sesiones específicas, este artículo ilumina la manera en la que el psicodrama y el grupo metabolizan elementos transferenciales al situar a los participantes en una obra improvisada.
