The experimental group demonstrated a more pronounced decrease in anxiety, exhibited lower levels of diastolic blood pressure (DBP), showed significantly lower levels of depression, higher levels of diabetes self‐management and self‐efficacy, and comparable levels of perceived social support compared to the control group.\n\n\nConclusions\nThe program has the potential to improve transitional care in older adults with diabetes, warranting further promotion.\n\n"]
